Presentation
- Sudden, painless, transient loss of vision in one eye, classically described as a "curtain" or "shade" moving across the visual field, typically lasting seconds to a few minutes (often cited as under 30 minutes) with complete recovery.
- History must clarify monocular versus binocular loss — patients (especially children) often cannot reliably distinguish; ask specifically whether they covered each eye during the episode.
- In patients over 50, ask directly about jaw claudication, scalp tenderness, headache, and other giant cell arteritis (GCA) symptoms.
- Risk factors: hypertension, diabetes, hyperlipidaemia, smoking, known carotid or cardiac disease, atrial fibrillation.
Findings
★ Signature Finding
Often a normal exam between episodes, since vision has typically returned to baseline by the time of evaluation — the diagnosis relies heavily on history.
- If examined during or soon after an episode: embolic material may be visible in a retinal vessel (a Hollenhorst plaque — a cholesterol embolus, classically lodged at an arteriolar bifurcation) or transient retinal whitening.
- Careful temporal artery exam (tenderness, nodularity, reduced pulse) and cardiovascular exam (carotid bruit, arrhythmia) when GCA or an embolic source is suspected.
Etiology & Mechanism
- Monocular (amaurosis fugax proper): most commonly embolic — from ipsilateral internal carotid artery atherosclerotic disease/dissection, or a cardiac source (atrial fibrillation, valvular disease).
- Vasculitic: giant cell arteritis causing inflammatory occlusion of posterior ciliary/ophthalmic artery branches — critical to identify given the risk of subsequent irreversible vision loss.
- Binocular transient visual loss: more typically reflects posterior (vertebrobasilar) circulation TIA or migraine with visual aura.
- Other/rarer causes: ocular ischaemic syndrome, hypercoagulable states, vasospasm, gaze-evoked amaurosis from a compressive orbital/optic-nerve lesion, hyperviscosity syndromes.

- Amaurosis fugax is functionally an ocular transient ischaemic attack and must be treated as a medical/ophthalmic emergency given the associated stroke risk.
- Any feature suggestive of GCA in a patient over 50 (jaw claudication, scalp tenderness, headache, unexplained weight loss/fever) — obtain ESR, CRP, and platelet count immediately and consider empiric high-dose corticosteroids plus urgent temporal artery biopsy.
- Recurrent episodes, or episodes with incomplete recovery, indicate a heightened stroke risk requiring urgent stroke-team-level evaluation.
Investigations
- Immediate ESR, CRP, and platelet count in patients over 50 (checking all three increases sensitivity for GCA).
- Urgent carotid imaging (duplex ultrasound, CT angiography, or MR angiography) for stenosis or dissection.
- Cardiac workup: ECG and echocardiography to evaluate for arrhythmia, valvular source, or thrombus.
- Brain MRI (including diffusion-weighted imaging) — a meaningful proportion of patients with transient monocular vision loss have been found to have silent acute brain infarcts on imaging despite being neurologically asymptomatic.
- Complete blood count to exclude anaemia or hyperviscosity states when clinically indicated.
Management
- Treat as an acute cerebrovascular event: refer for an expedited, stroke-protocol-level evaluation given the elevated short-term stroke risk.
- If GCA is suspected, do not delay corticosteroid initiation while awaiting biopsy — temporal artery biopsy can be performed after treatment has started, since biopsy positivity persists for a period afterward.
Treatment
- Antiplatelet therapy (e.g., aspirin) or anticoagulation, chosen according to the identified aetiology (e.g., anticoagulation for atrial fibrillation).
- Carotid endarterectomy or stenting for significant symptomatic carotid stenosis, in coordination with vascular surgery/neurology.
- High-dose corticosteroid therapy, started empirically, for suspected or confirmed GCA.
- Management of underlying modifiable vascular risk factors — blood pressure, lipids, glycaemic control, smoking cessation.
Follow-up
- Close short-term follow-up (days), coordinated with stroke/vascular teams, given the high near-term risk of stroke or permanent vision loss.
- Long-term secondary stroke-prevention management with primary care, neurology, and/or cardiology.
- For GCA, a prolonged steroid taper with ongoing monitoring of inflammatory markers and symptoms, co-managed with rheumatology.

Amaurosis fugax should always be treated with the same urgency as a cerebral TIA; missing an underlying giant cell arteritis can result in sudden, irreversible bilateral blindness that is largely preventable with timely corticosteroid therapy.
